Someone With Group, LLC Privacy Policy – on behalf of the PinnacleHealth HOPE Program

This Privacy Policy (“Policy”) describes how Someone With Group, LLC uses and protects your personally identifying information (“Personal Information”) when you are visiting the PinnacleHealth HOPE site: www.pinnaclehealthhope.org (the “Site”) and under what circumstances it may be disclosed. Continued use of the Site indicates your acceptance of this Privacy Policy.

Information We Collect:We may collect information about you from a variety of sources, including information that you provide to us directly and information we collect about you when you visit the Site.

Information You Provide To Us Directly : We collect Personal Information directly from you when you apply for a PinnacleHealth HOPE card (the “Card”), contribute to a Card account, or create a profile page. This information may include:

  • Mailing address
  • Email address
  • Username and password
  • Phone number
  • Social security number
  • Birth date
  • Future communications preferences
  • Information that you post as part of your profile

Some of the information we request will be voluntary and be identified as optional during the registration process. You may change or update your registration with us at any time by logging onto your account and choosing “Account Settings”. In connection with our current and planned electronic commerce services, “Contributors” will be asked to provide information, including credit card information, that is necessary to complete a contribution transaction you have requested. To protect you, Someone With Group, LLC does not store your credit card information.

Information we collect when you visit our Site, use our services, or browse our products: Like most wesites, we may also collect information indirectly from you when you visit our Site or use our services. Specifically, we use cookies, web beacons, log files, and other technologies to collect information about you when you visit our website. We use these technologies to monitor how our solution Site is used and to help personalize your user experience online. We also use these technologies to identify products and services that may be of interest to our customers overall and to you in particular. We do not store any of your Personal Information in cookies, web beacons, or log files. The following are examples of information we may collect using these technologies:

  • Email addresses that you share your story with for customer support

How We Use Your Information: We use the information that you provide us to offer and provide you with products and services. We also use the information to manage and enhance our website and the products and services that we offer. The following are ways that we use your information:

  • To apply for a Card
  • When you share your story through either a public or private account
  • To enhance your user experience on our website
  • To help us tailor our products and services to you specifically or for our customer base as a whole
  • To develop and to provide advertising tailored to your interests
  • To email you a newsletter highlighting products or services available on our website, notices about website posts (if requested), new products, and customer service information
  • To create and manage your account
  • To enforce our terms and conditions and to manage our website and business
  • To contribute to a Card account (if a Contributor)
  • To process your payment for funding a Card (if a Contributor)

We also may use the information we collect when you visit our site, use our services, and browse our products to enhance our customers’ experiences on our solution site, for product research, to troubleshoot problems with the website, analyze trends, and to comply with the law and law enforcement activities.

Sharing Information: We may share your Personal Information with our affiliates, business partners, or manufacturers, or third party service providers as necessary to process your order or provide services to you. Specifically, our business partners include hospitals or health services providers, our payment processing partners, consumer reporting agencies and other service providers used as part of the process of applying for or contributing to a Card account. We may also rely on third party service providers to:

  • Process your order
  • Host our website and deliver email communications
  • Analyze data

Third party service providers are only permitted to use your Personal Information to perform services on our behalf. We do not authorize our third party service providers to sell, share, or rent this information or to use it for any purpose not disclosed in this Policy.

In the future, we may share your Personal Information with other entities with like-minded interests. In general, we do not share non-aggregated personal information with third parties unless we have asked for and obtained your consent. We do share aggregated information that does not contain Personal Information, such as aggregated purchasing patterns, with third parties. We may also share your Personal Information, on occasion, with other trusted third parties who may wish to send you information about products and services that you may find of interest or for other reasons that are disclosed to us and that we believe to be appropriate and consistent with our commitment to prevent abuse of your Personal Information. We also may share aggregate purchasing information with manufacturers offering products on our website, with researchers or with our third party marketing partners.

We reserve the right to share your Personal Information to protect our rights and interests or the rights or interests of others, as required by law or order of a court or government agency, to enforce or apply our Terms and Conditions and other agreements. If we or some or all of our assets are acquired by another company, including through a sale in connection with bankruptcy, that company will possess the information collected by us, and it will assume the rights and obligations regarding your information as described in this Privacy Policy.

HIPAA: As part of the Card application process, your health services provider will disclose some health information to Someone With Group, LLC. Your health services provider will disclose the minimum necessary amount of your Protected Health Information to Someone With Group, LLC in order to facilitate your enrollment in the Someone With Group, LLC program including your registration for a debit card. Such information may include, but is not limited to, your name, Social Security Number, address and date of birth. The terms of this disclosure are described in the Notice of Privacy Practices provided by your health services provider.

Interest Based Advertising: We may partner with third-party service providers to serve ads on our behalf on non-affiliated third party sites. Some of those ads may be personalized, meaning that they are intended to be relevant to you based on information we or the online advertising network serving the ad collect about your online activities here and elsewhere over time. For example, anonymous information collected across multiple sites that you visit over time may enable the ad network to predict your preferences and show you ads that are most likely to be of interest to you. Please note that we do not share any information that identifies you personally with the third party service providers who serve ads on our behalf.

Changes to this Privacy Statement: We reserve the right to change this Policy from time to time consistent with applicable law. We will always identify the effective date of the Policy at the top of the Policy. Changes to our Policy shall become effective as of the date they are posted to the website.
